Medications

Students should not carry medications with them during the school day, but rather they should be brought to the Nurse’s Office in the original prescription container with a note signed by a parent (for information  see p. 4 of this web site).The exception is asthma inhalers and Epi-Pens (for allergic reactions).  These may be carried provided there is a doctor’s note on file in the Nurse’s Office.  There is a nebulizer and peak-flow meter) available in the Nurse’s Office (please send in the student’s mouthpiece and medication).

 

Gym Excuses

Students who need to be excused from Gym class should bring a note from a parent to the Nurse’s Office before school on that morning.

 

Physicals

These are required for all entering students as well as those who will be participating in sports.  These must be turned in to the Nurse before the first day of school or before the first practice in the case of sports. A sports physical is required every year after June 1st.

 

Immunizations

Minimum requirements:

DTP (Diphtheria, Tetanus, Pertussis)—5 doses and then a Tetanus booster every ten years (unless the fourth was given after the fourth birthday)

 

Polio—4 doses unless third dose is given after the fourth birthday

 

MMR (Measles, Mumps, Rubella)—2 doses

 

Hepatitis B—3 doses

 

TB test (PPD)—should have been completed within the last 12 months

 

Varicella Vaccine  (chicken pox) or documentation of having had the disease. Which may be provided by the physician or parent (date of the disease and age at that time)
